Hematopoietic stem cell transplantation (HSCT) is the transplantation of blood stem cells derived from the bone marrow (in this case known as bone marrow transplantation) or blood. Stem cell transplantation is a medical procedure in the fields of hematology and oncology, most often performed for people with diseases of the blood, bone marrow, or certain types of cancer.

With the availability of the stem cell growth factors GM-CSF and G-CSF, most hematopoietic stem cell transplantation procedures are now performed using stem cells collected from the peripheral blood, rather than from the bone marrow. Collecting peripheral blood stem cells provides a bigger graft, does not require that the donor be subjected to general anesthesia to collect the graft, results in a shorter time to engraftment, and may provide for a lower long-term relapse rate.

Hematopoietic stem cell transplantation remains a risky procedure with many possible complications; it has traditionally been reserved for patients with life-threatening diseases. While occasionally used experimentally in nonmalignant and nonhematologic indications such as severe disabling auto-immune disease and cardiovascular disease, the risk of fatal complications appears too high to gain wider acceptance.

why can't recipients of bone marrow transplants be under anesthesia during the transplant?
Q. I have been told that a recipient of a bone marrow transplant can't be under anesthesia, but wasn't told why. And I can't find it anywhere. Does any one know?

A. I never heard of that, but no, we do not give anesthesia for this. I thought it was just a matter of getting an IV. The donors get anesthesia, though. Donating involves lots of punctures into your hip bones with a very big needle.

What exactly is a bone - marrow transplant?
Q. How many bones inside the skeletal system need the marrow to be replaced traditionally? What are the different procedures involved in the bone marrow transplant surgery? What are the downfalls of having this procedure done? Lastly, why do phlebotomists check different blood types (in detail - i know the basics) before the surgery? Does it matter if the marrow doner is on any medications like anti-hypertensives or anti-hyperlipidemics or anti-diabetic medications? I am wanting to know, because my aunt is in need of a bone-marrow transplant, and is cured of her leukemia after a ton of chemo-therapy. This is really important to me.

A. 1. ALL the bones that contain marrow need the marrow replaced. If all existing marrow is not removed, the patient's remaining marrow will fight the transplant. 2... But, its not surgery, in the way most people think of surgery. Instead of cutting into her and removing the marrow, they will use high dose chemo and full body radiation to kill the bone marrow and all white blood cells in the circulating blood. After that, the donated marrow or stem cells are infused via IV, much the same as any other blood product would be. From the start, she will be admitted to the hospital, go through a few days of full body radiation, then a few days of chemo. Then she will receive the transplant. She will be in the hospital for about 6 weeks… [cont.]

Q. I think it had to do with boys in a prep school and one pushing another down the stairs and breaking his leg or something? Help me out here! Needed a bone marrow transplant possibly.

A. I think you mean A Separate Peace by John Knowles. My son read it last year in 8th grade. It takes place at a New England prep school like Knowles alma mater, Exeter. It was not my son's favorite book, but it was very thought provoking, he still talks about it.
